and I am Matthew's Mom. -
0:19 - 0:21>>[Therapist]: You see yourself?
(Laughter) Look at you. -
0:21 - 0:24>>[Mom]: He has a really funny sense
of humor. He's very visually stimulated. -
0:24 - 0:26You know initially when you
have a child that's nonverbal... -
0:26 - 0:29...people often ask, "well you know
how do they communicate?" -
0:29 - 0:32I always say my goal, yes, I want
Matthew to be able to talk. -
0:32 - 0:36But, I had to broaden my horizons,
and you know what.. -
0:36 - 0:37..you want him to be
able to communicate. -
0:37 - 0:40>>[Therapist to Matthew]:
Do you want this? Do you want this? -
0:41 - 0:43Do you want your drink?

0:43 - 0:45[Therapist interview] I started working
with Matthew several years ago. -
0:47 - 0:52I'm Sarah Bethshaw and I'm the Speech
Language Pathologist here, at Trellis School. -
0:54 - 0:56When he wasn't talking and he
wasn't able to imitate at that time, -
0:56 - 1:02I decided it was time to implement another functional mode of communication for him.

1:02 - 1:08So we started with a picture exchange
communication system, which is very widely known. -
1:08 - 1:11[Therapist to Matthew]:
I want...eat...popcorn. -
1:11 - 1:15[Therapist interview]: That was,
probably the first time in his life -
1:15 - 1:18he had functional communication
across a variety of contexts... -
1:18 - 1:20..um..that really worked for him.

1:20 - 1:22>>[Mom]: He was doing really
well with PECS and as a result, -
1:22 - 1:27that's why we asked for, could we possibly
use an assisted technology device -
1:27 - 1:29to help him with communication.

1:29 - 1:31>>[Communication Device]: Cookie

1:31 - 1:33>>[Therapist to Matthew]: Here you go.

1:36 - 1:38Yummm!

1:38 - 1:43[Therapist interview]: The beauty of this device
is the capacity for him to communicate is....vast. -
1:43 - 1:47You wanna start with the core.
We use core vocabulary words. -
1:47 - 1:52Core vocabulary words are words like
eat, drink, stop, play, go, more. -
1:52 - 1:54>>[Communication Device]: Eat.
>>[Therapist to Matthew]: Here ya' go. -
1:54 - 1:57[Therapist interview] It speaks so he speaks.

1:57 - 2:01>>[Communication Device]: Drink
>>[Therapist to Matthew]: Drink! Here's your drink! -
2:02 - 2:03You want to play?

2:03 - 2:07[Therapist interview]: You find places where
they're most motivated and you put the core into it. -
2:08 - 2:11>>[Communication Device]: Go.
>>[Therapist to Matthew]: You want to go? Let's go! -
2:13 - 2:17>>[Communication Device]: Go.
>>[Therapist to Matthew]: Good Job! Goooo! -
2:17 - 2:19>>[Another therapist]:
Crash them! Wooooo! -
2:19 - 2:21>>Therapist to Matthew]:
Hey my turn, can I have a sip? -
2:21 - 2:23[Therapist interview] Today he
was doing well with the word, turn. -
2:23 - 2:25>>[Communication Device]: Turn

2:26 - 2:29>>Therapist interview]: It wasn't taking as
much prompting as it sometimes does. -
2:29 - 2:31So then I wanted to increase the difficulty

2:31 - 2:37I add words, while I'm working with him,
so I want to add the word "my", -
2:38 - 2:40so I just went ahead and added it..

2:40 - 2:43So now, I have the word, "my,"
so I can do, "my turn." -
2:43 - 2:48It increases the amount of hits you're
gonna push and....it's a little bit harder. -
2:48 - 2:53>>[Communication Device]: My...Turn
>>[Another Therapist]: My Turn....Goood! -
2:53 - 2:55Here you go, carry this.

2:55 - 2:58>>[Therapist interview]: These devices
on a typical day are with him all the time. -
2:58 - 2:59>>[Communication Device]: Come.

2:59 - 3:01>>[Therapist interview]: A speech
therapist needs training on it... -
3:01 - 3:04...then the family also
needs training on it. -
3:04 - 3:10One of the best things about this device is it's
dynamic, so it's changing with us all the time. -
3:10 - 3:15So, we can work with the displays and make
it fit exactly what we're trying to teach him. -
3:15 - 3:21>>[Communication Device]: Eat....Cookie
>>[Therapist]: Eat cookie! Good job! -
3:21 - 3:25[Therapist interview]: That was really
exceptional. He said, "Eat Cookie." -
3:25 - 3:26He said the whole phrase
completely and independently. -
3:26 - 3:27And that was four hits.

3:27 - 3:31So the first time today, when he was eating
cookies, I've never seen him eat those cookies. -
3:31 - 3:33I could tell he loved those cookies.

3:33 - 3:36So in the moment, I want "Yum" on there.

3:36 - 3:38[Therapist to Matthew]:
Yeah, you want to say "Yum!" -
3:38 - 3:41>>[Communication Device]: Yum.
>>[Therapist]: The cookies are yummy! -
3:41 - 3:46So now in this moment, we've activated...
we've started using two comments.... -
3:46 - 3:50..which is something we're working with
Matthew on, saying "Like" and "Yum." -
3:50 - 3:54And, that's really important for him not
to just be requesting, using the device, -
3:54 - 3:57but using it for a variety of
communicative intents. -
3:58 - 4:00[Therapist to Matthew]: What do you want?
>>[Communication Device]: Cookie. -
4:00 - 4:02>>[Therapist]: Cookie!

4:02 - 4:06[Therapist interview] And the next time he
ate cookies, he was very pleased with that. -
4:06 - 4:08[Therapist to Matthew]: You wanted
to say "Yum," let me teach you. -
4:08 - 4:12Yum starts at the top.
Eyes on. Look, this is "yum." -
4:12 - 4:15>>[Communication Device]: Yum.
>>[Therapist]: Yummmmm! -
4:15 - 4:16You wanted to say yum!

4:16 - 4:19>>[Communication Device] Yum. Yum.
>>[Therapist]: Yummmmm! -
4:19 - 4:21[Laughter]

4:21 - 4:25[Therapist interview] To me
the goal for Matthew is to -
4:25 - 4:29confidently, functionally,
and proudly communicate. -
4:29 - 4:34>>[Mom]: Through giving him a way to communicate,
we've decreased behaviors tremendously. -
4:34 - 4:37I would say start off with the picture
exchange communication system. -
4:37 - 4:41Once he got to having four to six
pictures on the front of his PECSbook-- -
4:41 - 4:45it's like, you know what, we can
take it up another level. -
4:45 - 4:48As his mom, you know my expectation
have always been really high and I'm like: -
4:48 - 4:50we're going to maximize
Matthew's potential. -
4:50 - 4:53He can, you know, and we truly
believe that he will be able to talk. -
4:53 - 4:57>>[Therapist interview]: We will
never say that a child won't talk. -
4:57 - 5:01He absolutely may talk,
but right now, he has this. -
5:01 - 5:06>>[Communication Device]: Like.
>>[Therapist]: You like music? Yay! Let's do music. -
5:06 - 5:09>>[Mom]: Just because he's non-verbal,
does not mean he doesn't have a lot to say. -
5:09 - 5:13>>[Communication Device]: More.
>>[Therapist]: Moorrree. That's more. -
5:13 - 5:15>>[Mom]: Oh wow!
It's given him a voice. -
5:15 - 5:18>>[Communication Device]: Yum. Yum.
