WEBVTT 00:00:12.423 --> 00:00:15.344 Good afternoon. How was the Venlo burger? 00:00:15.754 --> 00:00:17.029 Good? 00:00:17.029 --> 00:00:18.905 Let me ask you some personal questions: 00:00:18.905 --> 00:00:21.743 which one of you is in a romantic relationship? 00:00:22.144 --> 00:00:23.674 Please, raise your hand. 00:00:24.553 --> 00:00:27.008 (Laughter) 00:00:27.485 --> 00:00:30.551 I'm not hitting on you, guys. Trust me, no. 00:00:33.405 --> 00:00:36.754 Which one of you is lucky enough to have a couple of friends around? 00:00:37.386 --> 00:00:39.215 Again, a lot of people. 00:00:39.215 --> 00:00:43.672 So, I should conclude you all have a lot of people around you. 00:00:45.297 --> 00:00:51.008 So, which one of you knows someone who suffers from mental distress, 00:00:51.974 --> 00:00:54.481 or even a mental disorder? 00:00:56.585 --> 00:01:00.028 It's a lot harder to raise your hand now, isn't it? 00:01:01.425 --> 00:01:04.726 One out of four people in the Netherlands suffers from mental distress 00:01:04.726 --> 00:01:07.428 and one out of ten people suffers from a condition 00:01:07.428 --> 00:01:10.194 that we could diagnose as a mental disease. 00:01:10.924 --> 00:01:12.817 Please, look around you. 00:01:13.938 --> 00:01:17.639 My talk is about you and the people in your lives. 00:01:19.636 --> 00:01:24.614 I want to show you how our relatively non-severe mental problems 00:01:24.614 --> 00:01:27.637 can go from bad to worse very quickly, 00:01:27.637 --> 00:01:30.686 and how often we create these situations ourselves. 00:01:31.439 --> 00:01:33.318 I also want to show you 00:01:33.318 --> 00:01:37.579 how our healthcare system fails to address complex problems 00:01:37.579 --> 00:01:41.940 and how we can use the power of suffering in order to evolve. 00:01:43.527 --> 00:01:45.469 I want you to meet Jane. 00:01:45.931 --> 00:01:47.530 Now, we all know Jane. 00:01:47.530 --> 00:01:52.448 She could be your neighbor, your relative, your average colleague. 00:01:53.909 --> 00:01:56.151 Jane is a 40-year-old woman, 00:01:56.450 --> 00:01:59.970 and she's married, has two children, a son and a daughter, 00:01:59.970 --> 00:02:02.666 and she works as a secretary in a large company. 00:02:03.079 --> 00:02:05.141 One day, Jane doesn't show up for work. 00:02:05.141 --> 00:02:07.032 She was feeling tired for a while, 00:02:07.032 --> 00:02:12.372 and she reports to her family doctor and he advises her to take a good rest. 00:02:12.372 --> 00:02:15.480 So, that's what she does: she goes home and she takes a good rest, 00:02:15.480 --> 00:02:18.637 and this rest takes up to four weeks. 00:02:19.356 --> 00:02:22.660 After four weeks, she gets back to work. 00:02:23.130 --> 00:02:28.604 Six months later, she drops out again: same symptoms, but worse. 00:02:29.228 --> 00:02:34.197 Again, she reports to her family doctor and he diagnoses her with burnout. 00:02:35.092 --> 00:02:37.870 He prescribes her antidepressant medication 00:02:37.870 --> 00:02:41.201 and, again, advises her to take a good rest. 00:02:41.201 --> 00:02:44.122 This time, this rest takes up to four months. 00:02:45.703 --> 00:02:47.725 After these four months, 00:02:48.212 --> 00:02:51.662 she goes back to work, still on medication. 00:02:51.662 --> 00:02:55.902 Two months later, she drops out again, third time within a year. 00:02:55.902 --> 00:02:57.545 Severe depression. 00:02:58.225 --> 00:03:00.542 She's referred to specialized mental healthcare 00:03:00.542 --> 00:03:02.023 for a very intensive treatment, 00:03:02.023 --> 00:03:04.763 and, after one year of intensive treatment, 00:03:04.763 --> 00:03:08.283 she ends up at home, losing her job permanently, 00:03:08.283 --> 00:03:10.040 still depressed. 00:03:11.081 --> 00:03:12.957 So, what's going on? 00:03:14.463 --> 00:03:17.019 Why isn't her medication working? 00:03:18.031 --> 00:03:20.841 Why isn't the therapy working? 00:03:22.550 --> 00:03:27.115 The real problem that Jane is facing is that she has an addicted son 00:03:27.934 --> 00:03:30.384 stealing from her every day, 00:03:30.903 --> 00:03:35.866 manipulating her every single day, 00:03:38.214 --> 00:03:42.272 lying to her, threatening her. 00:03:42.272 --> 00:03:46.604 So, every time she was sent home by her family doctor to take a rest, 00:03:46.604 --> 00:03:49.112 she was sent straight into hell. 00:03:50.925 --> 00:03:52.644 The question that comes to mind is: 00:03:52.644 --> 00:03:57.175 why didn't Jane tell her family doctor about her addicted son? 00:03:57.175 --> 00:04:01.186 She only asked him to help her with her depression. 00:04:02.985 --> 00:04:06.515 But, on the other hand, the family doctor knows her son. 00:04:07.258 --> 00:04:10.835 In fact, he knows the whole family, and he also knows her son is addicted. 00:04:10.835 --> 00:04:15.978 How come he's only focusing on the symptoms Jane is presenting? 00:04:16.897 --> 00:04:19.795 And how come the specialized mental healthcare 00:04:19.795 --> 00:04:22.560 is doing exactly the same thing? 00:04:23.586 --> 00:04:26.052 That's because they're doing their job. 00:04:28.286 --> 00:04:30.856 The family doctor is doing his job well, 00:04:30.856 --> 00:04:34.996 focusing on the symptoms his patient is presenting, and treating them. 00:04:34.996 --> 00:04:39.225 scaling up, as does the specialized mental healthcare, 00:04:39.225 --> 00:04:41.316 following protocol 00:04:41.316 --> 00:04:45.831 and treating her with so-called evidence-based treatment. 00:04:47.398 --> 00:04:51.437 It is however the system that doesn't allow them 00:04:52.887 --> 00:04:56.032 to focus on the main source of the problem. 00:04:56.787 --> 00:04:58.672 The system is broken. 00:04:59.456 --> 00:05:06.413 It is the system that does not address complex situations like Jane's adequately. 00:05:07.858 --> 00:05:14.888 It is this system failure that made me as a mental professional healthcare worker 00:05:15.940 --> 00:05:21.419 think about a drastic change in the way we help people like Jane. 00:05:22.308 --> 00:05:26.169 And so, I want to introduce you to the M.A.D. approach. 00:05:26.539 --> 00:05:29.911 "M.A.D." stands for motivation analysis diagram, 00:05:30.651 --> 00:05:34.891 and M.A.D. is based on the principle that the level of suffering 00:05:35.258 --> 00:05:40.191 influences our motivation to take responsibility for our own recovery, 00:05:40.191 --> 00:05:42.755 and taking this responsibility 00:05:43.150 --> 00:05:47.073 is a fundamental element in any effective treatment. 00:05:49.260 --> 00:05:53.038 In order to show you that it is a real diagram, 00:05:53.038 --> 00:05:54.441 this is how it looks like. 00:05:54.441 --> 00:05:59.251 It tries too far to fully explain the model in full detail, 00:05:59.251 --> 00:06:01.936 but I will show you its main principles. 00:06:02.351 --> 00:06:04.520 And the first principle of the M.A.D. approach 00:06:04.520 --> 00:06:07.511 is that the degree of suffering influences motivation. 00:06:07.511 --> 00:06:12.732 As you can probably imagine, if my level of suffering is too low, 00:06:13.554 --> 00:06:17.632 then I probably won't be motivated to change my behavior 00:06:17.632 --> 00:06:19.509 or make other choices. 00:06:20.171 --> 00:06:24.932 On the other hand, if my level of suffering is too high, 00:06:25.283 --> 00:06:28.602 chances are that I'm not able to change, 00:06:29.343 --> 00:06:31.911 due to too much pain or pressure. 00:06:34.683 --> 00:06:38.068 So, if I can influence your level of suffering, 00:06:38.068 --> 00:06:41.458 then I can also influence your behavior. 00:06:43.234 --> 00:06:45.373 In Jane's case, 00:06:45.373 --> 00:06:49.165 the family doctor and the specialized mental healthcare 00:06:49.165 --> 00:06:54.508 decreased the level of suffering by treating her symptoms, 00:06:55.725 --> 00:06:59.184 but therefore, they also decreased her motivation 00:06:59.184 --> 00:07:01.651 to address the main source of the problem. 00:07:02.913 --> 00:07:09.960 So, influencing the level of suffering is a key element in the M.A.D approach. 00:07:12.296 --> 00:07:14.295 The second main principle 00:07:14.615 --> 00:07:20.175 is that M.A.D. focuses on the main source of the problem, 00:07:20.175 --> 00:07:23.218 instead of the symptoms that are presented. 00:07:24.276 --> 00:07:27.720 Again, in Jane's case, we stopped her treatment, 00:07:28.277 --> 00:07:33.505 we also stopped her medication, and all we did was follow up. 00:07:33.505 --> 00:07:39.486 We only monitored the level of suffering rising again. 00:07:39.866 --> 00:07:43.477 Eventually, she opened up, she told us about her addicted son. 00:07:43.477 --> 00:07:46.607 So, that's what we did: we treated her son's addiction, 00:07:46.607 --> 00:07:49.736 we gave him the attention and care that he needed, 00:07:49.736 --> 00:07:53.008 and we also provided him with another place to live. 00:07:53.298 --> 00:07:55.122 Within four months, 00:07:56.877 --> 00:08:01.211 Jane's symptoms cleared, without any additional treatment. 00:08:03.159 --> 00:08:06.497 The M.A.D. approach has been proven successful in the past three years 00:08:06.497 --> 00:08:08.408 within my company, 00:08:08.408 --> 00:08:10.174 and as a comparison, 00:08:10.786 --> 00:08:15.216 up to 85% of the people who are treated in regular addiction healthcare 00:08:15.216 --> 00:08:19.331 will fall back in their former habits within the first year. 00:08:19.331 --> 00:08:23.214 Eighty-five percent will fall back. 00:08:24.899 --> 00:08:26.498 Using the M.A.D. approach, 00:08:26.498 --> 00:08:30.327 we are able to reduce this fallback to only 10%, 00:08:30.327 --> 00:08:35.199 and just imagine the personal benefits for our patients. 00:08:36.590 --> 00:08:42.010 Imagine the social benefits, or even the financial benefits. 00:08:43.936 --> 00:08:46.591 It is my personal mission for the upcoming years 00:08:46.591 --> 00:08:49.860 to make this M.A.D. approach trainable and therefore accesible 00:08:49.860 --> 00:08:52.948 to every professional healthcare worker. 00:08:53.801 --> 00:08:57.748 But you also can start using the M.A.D. approach today, 00:08:58.030 --> 00:09:02.227 helping your relatives, helping your loved ones or colleagues. 00:09:02.542 --> 00:09:07.054 And in order to do so, I have three recommendations for you. 00:09:07.810 --> 00:09:11.172 First one is: recognize suffering. 00:09:11.533 --> 00:09:15.009 In order to do so, keep asking questions. 00:09:15.593 --> 00:09:17.081 Again, it's no rocket science, 00:09:17.081 --> 00:09:20.251 but try to figure out what is really going on here, 00:09:20.251 --> 00:09:22.594 instead of jumping to conclusions. 00:09:23.282 --> 00:09:26.389 Try to detect the main source of the problem. 00:09:27.013 --> 00:09:31.621 The second one is: stop focusing on what people want. 00:09:32.853 --> 00:09:35.907 Start focusing on what they really need. 00:09:36.301 --> 00:09:38.925 These two things are not the same. 00:09:41.173 --> 00:09:45.541 The third one is that nobody likes to suffer, it hurts, 00:09:46.862 --> 00:09:49.172 but instead of reducing the pressure, 00:09:49.172 --> 00:09:54.213 you can use it as a powerful tool for you to evolve. 00:09:55.033 --> 00:09:59.414 So, next time, tomorrow, on Monday, back at work, 00:09:59.414 --> 00:10:01.624 next time you meet your Jane, 00:10:01.624 --> 00:10:04.524 I want you to remember these recommendations. 00:10:05.854 --> 00:10:07.444 Thank you for listening. 00:10:07.444 --> 00:10:09.903 (Applause)
